1.2congratulating Dr. Tori Bahr for receiving the 2024 Humanitarian Award. 1.3WHEREAS, the Humanitarian Award is presented annually by the Minnesota Medical 1.4Association and Copic Insurance to a physician who demonstrates meaningful work in improving 1.5health care in their local community; and 1.6WHEREAS, Dr. Bahr is an internal medicine-pediatrics physician recognized for her 1.7dedication to medically complex children and their families by providing holistic, coordinated care; 1.8and 1.9WHEREAS, Dr. Bahr recognized the challenges faced by her pediatric patients transitioning 1.10to adult care and established a health care transition process within her practice. She saw the potential 1.11to expand this approach in a sustainable and equitable manner; and 1.12WHEREAS, Dr. Bahr launched an initiative to improve the health care transition process 1.13for youth with medical complexity in the state of Minnesota, addressing critical service gaps across 1.14pediatric and adult health systems, as an unpaid contribution beyond her clinical duties; and 1.15WHEREAS, Dr. Bahr's advocacy includes organizing discussions to address systemic barriers 1.16in transitions in collaboration with the Minnesota Rare Disease Advisory Council, the Minnesota 1.17Chapter of the American Academy of Pediatrics, and the Minnesota Department of Health; and 1.18WHEREAS, Dr. Bahr has also contributed to medical education surrounding transition to 1.19adult care. She developed the MedPeds Transition elective for the University of Minnesota Medical 1.20School, and organized the Twin Cities Transition Quality Improvement Collaborative with providers 1.21from multiple institutions to help young adults with medical complexity navigate their first 1.22hospitalization outside of a children's hospital; and 1.23NOW, THEREFORE, BE IT RESOLVED by the Senate of the State of Minnesota that it 1.24congratulates Dr. Tori Bahr for her significant contributions to her community, and work to improve 1.25these critical health care conditions. 1.26BE IT FURTHER RESOLVED that the Secretary of the Senate is directed to prepare an 1.27enrolled copy of this resolution, to be authenticated by the Secretary's signature and that of the 1.28Chair of the Senate Rules and Administration Committee, and transmit it to Dr. Tori Bahr.
